Welcome to FollowMe™. This site is owned by Access Strategies, Inc. (ASI), a California-based healthcare consulting and patient advocacy company established in 1990.
FollowMe is a Personal Health Record (PHR) created so that individuals can store and access their important health care information at any time and anywhere Internet Access is available.

Information Collection and Use
FollowMe does not collect, rent or sell your personally identifiable information. This includes, but is not limited to, mailing addresses, telephone or fax numbers and email addresses. Your contact information is stored by FollowMe in a safe and secure manner at remote servers that are protected 24/7.
FollowMe may offer users, from time to time, the opportunity to register for surveys and clinical trials and other research related activities. We will do this by requesting that users, at the time of registration, complete a form authorizing the release of contact information.
FollowMe adheres to the principles and guidelines of the Health Insurance Portability and Accountability Act (HIPAA). The privacy provisions of the federal law, the Health Insurance Portability and Accountability Act of 1996 apply to health information created or maintained by health care providers who engage in certain electronic transactions, health plans and health care clearinghouses. The Department of Health & Human Services (DHHS) has issued the regulation, Standards for Privacy of Individually Identifiable Health Information” applicable to entities covered by HIPAA. The Office for Civil Rights (OCR) is the Department component responsible for implementing and enforcing the privacy regulation. FollowMe strives to maintain and exceed the federal guidelines regarding the privacy and security of Personally Identifiable Information.
The information stored on FollowMe belongs to the individual subscriber. Only by permission of the user/subscriber may information, including passwords, be shared with other parties.

Log Files
We use IP addresses to analyze trends, administer the site; track user’s movement and gather broad demographic information for aggregate use. IP addresses are not linked to personally identifiable information.
Sharing
We may share aggregate demographic information with our partners and sponsors. This is not linked to any personal information that can identify any individual person. (For example, we might collect the number of users in a particular state or the most common types of Internet Service Providers. But this information is not tracked to individual users.)
Security
FollowMe takes every precaution to protect users’ information. When users submit sensitive information via the website, your information is protected both online and off-line.
When our registration form asks users to enter sensitive information (such as a credit card number and/or social security number) that information is encrypted and is protected with the best encryption software in the industry – SSL. While on a secure page, such as our order form or your medical record forms, the lock icon on the bottom of Web Browsers such as Netscape Navigator and Microsoft Internet Explorer becomes “locked”, as opposed to un-locked, or open, when you are just surfing. While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user-information off-line. The servers we use to store personally identifiable information are located in a professionally managed co-location facility with state of the art security measures.

If we decide to change our privacy policy, we will post those changes on our homepage so that our users are always aware of what information we collect and how we use it.
